Lesson 5.2

What makes a good check-in form

Loom walkthrough coming for this lesson. The written version below is complete.

A check-in form is about gathering data: a mix of objective measures (weight, photos, completion) and subjective feedback (how they feel, what got in the way).

The rules

  • Under 3 minutes to fill out. If it's a chore, they'll skip it or rush it.
  • Make it easy. Drop-downs, tick boxes and sliding scales ("Rate your energy this week from 1 to 5") wherever you can.
  • Automate what you can. The client just does the work, and you do the coaching.
  • Same rules as the intake (Module 2). If you never use an answer, cut the question. If you keep asking the same follow-up, add it.

An example form, and why each question earns its place

For female clients, add one more question: where are you in your cycle this week, and are you on hormonal contraception? You read the scale through that answer (Modules 3.10 and 6.3).

What makes a good check-in from the client's side

On time, honest, and complete. Consistent photos (same lighting, same time of day, same poses) make visual progress readable. Teach this at onboarding and reinforce it by actually using what they send.
